Cocoon doesn't support xsl:output. You have to use <?cocoon-format
type="text/html"?>, and setup the correct DTD in your
cocoon.properties file. BTW, everything is ready for HTML, so you
shouldn't have anything to do, provided you insert the PI above, via
<xsl:processing-instruction>
