It's proving very tedious to trace and logically document all the code paths.  In the spirit
of keep it simple, how about in the block where {{httpResponse.sendError}} is invoked:  if
the response is going to be 401 but the auth handler didn't set the expected WWW-Authenticate
header that I convert it to a 403?  If nothing else, it's a failsafe for future bugs in auth
handlers.  I'll post a patch shortly.
