I've done this in the past to accomplish this:

<Location />
Order deny,allow
Deny from all
Allow from x.x.x.x
Allow from y.y.y.y
</Location>

<Location /maintenance_pages/>
Order deny,allow
Allow from all
</Location>

ErrorDocument 403 /maintenance_pages/index.html
#########

Where x.x.x.x and y.y.y.y would be IPs you would like to allow access 
during maintenance.  It may be ugly, but it works.
