Fixed at http://svn.apache.org/r1607120

New {{Continuation}} instances go to the end of the queue now in the case the {{MAX_QUEUED_CONTINUATIONS}}
kicks in. This allows the queue to be cleared first until taking it up again where the current
{{Continuation}} left.
