The cache system works with client acknowledgment disabled (by caching at most the number
of simultaneousness requests) and also with client acknowledgment enabled (by caching the
items not acknowledged yet by the client).
To prevent memory consumption problems there is a configurable (in BoshBackedSessionContext#maximumSentResponses)
hard limit that is enforced along the other limits.
