Patch for decrypting AES-192/256

AES has always a block-size of 128 bits, therefore we need take the keysize of
128, 192 or 256 bits into account.
Part of the patch fixes a wrong usage of the bit-sizes, i.e. the IV has to be
calculated by the block size (128 bits) whereas the encrypted key needs to use
the key size (e.g. 256 bits).

I'll try to provide a few more test files with other different encryption
settings and haven't tested document encryption at all ...
