These changes (which should be in 0.16) improve the 10 headers case from 55,000 Msg/s to about
80,000 Msg/s.

They address some inefficiencies in the Field Table implementation by avoiding most unnecessary
They remove the most obvious unnecessary broker copies of FieldTables (in the Exchange interface)

There is still some way to go though.
