Like the Qpid Java Client, this implementation does not send channel.flow-ok.

Hmmm - this seems more like a bug in the Java client than a desirable feature...  I'm not
sure what other 0-8/9/9-1 brokers do (do I remember that RabbitMQ actually doesn't implement
channel flow at all) ... but it still seems a little weird to be deliberately non-compliant
with the spec.
