Thank you both for your comments, they were very helpful.  Here's an example of the syntax
I ultimately went with (in case any SQL Server guru wants to tell me how wrong it is):
MERGE INTO [testTable] AS _target USING ( VALUES ( 1, 2 ) ) AS _source ( k, v )
ON _target.k = _source.k
WHEN MATCHED THEN UPDATE SET _target.v = _source.v
WHEN NOT MATCHED THEN INSERT ( k, v ) VALUES ( _source.k, _source.v );
I've attached two patches; one that includes the conditional merge functionality, and one
that doesn't.  Feel free to merge whichever you are more comfortable with.
