I use struts version 2.0.14 and I have the same problem. Default-aciton-ref doesn't work.

My configuration:

<constant name="struts.action.extension" value="html,,"/>
...
<package name="online" extends="default" namespace="/">
    <default-action-ref name="index"></default-action-ref>
    <action name="index" class="startPageAction" method="execute">
        <result>/WEB-INF/pages/index.jsp</result>
    </action>
</package>

Now when I type http://localhost:8080/myApplicationName/ browser raport HTTP Status 404.

I tried to use version 2.1.2 and there default-aciton-ref works with this configuration. 
But, because of other issues I cannot use this version :(
