I already started improving the outline view before this issue was created.
The result of that work is now checked in. I suggest that you review the improvements 
and we continue the discussion at that issue UIMA-1179. I will also attach a screenshot
to that issue, so you do not have to build the Cas Editor to see the changes.

the annotation schema is displayed as a tree structure displaying the name of each type in
a hierarchy.  Each node in the tree displays the name of the type followed by a number corresponding
to the number of annotations in that document of that type.  When you click on a node in the
tree it provides you a context sensitive menu that gives you an option to create a new annotation
for the currently highlighted text (if any) followed by options for selecting the annotations
of that type (one menu item per annotation).
