I checked in a change to several things, to support a higher fidelity of switching cas loaders
and also managing the cas locking (preventing calling cas.reset() within an annotator.  With
these, I got the above test case to work. 

Peter/Matthias, it would be greate if you could please test the v2 trunk (I haven't done anything
yet with v3 for this). 

The fix I hope works for the various error cases, with various Flow Controllers / CAS Multipliers,
etc., but there are lots of special cases, so I may have missed some (hence, the need for
testing :) beyond our test cases).
