ehm, not sure if this helps but have you tried a different order of the
attributes?


<xsl:stylesheet version="1.0"
 xmlns:xsl="http://www.w3.org/1999/XSL/Transform"
  xmlns:fo="http://www.w3.org/1999/XSL/Format">

I know it looks the same (except for the order) but perhaps fop needs the
attributes 
ordered like this?
