Current Visual Studio compilers fail to replace the
ROTATE_LEFT macro with a rotl machine instruction.
The attached patch tells VS what to do.

Save a couple of cycles on MD5 compiled with newer
Microsoft compilers (VS2003 ff.).

* crypt/apr_md5.c
  (ROTATE_LEFT): tell newer Microsoft compilers to 
  replace this macro with a single machine instuction.
