Based on discussion (with many people, Stu included) at the Digg hackathon, we should do this:

1. Do local indexes for low-cardinality data, including api changes to expose this.

Among other concerns, Stu mentioned that there might be some indexes with such low cardinality
that an index could be counterproductive. We agreed that in these cases we should still require
that the user declare that they'd like an index, but avoid actually writing the index until
it would be useful.

2. For distributed indexes we should build coprocessors/triggers/callbacks (name TBD) as a
separate project (and separate JIRA).
