I had a closer look a the problem and found that it is due to the HTML element having attributes
defined in a namespace ("<html xmlns="http://www.w3.org/1999/xhtml" xml:lang="en" lang="en">").

Having attributes without explicit namespace works fine (<html lang="en">)
