I'm not sure it makes the difference but I always precede the relative path
with the "file:" string like this :

<fo:external-graphic src="file:ecu.jpg"/>

or

<fo:external-graphic >
  <xsl:attribute name="src">
    <xsl:text>file:</xsl:text>
    <xsl:value-of select="imagepath"/>
   </xsl:attribute>
</fo:external-graphic>

and it works
